Job summary

Qualifications

• BSN completed and RN licensed in the appropriate state with no disciplinary action.
• 3–5 years of acute care RN experience.
• Must live in OH, KY, FL, AZ, TX.
• Proficient in Microsoft Office applications.
• Managed care experience.
• Patient-centered care mindset.

Responsibilities

• Deliver telephonic, patient-centered care in a structured call-center environment.
• Provide medication education, including administration techniques, side effects and adherence support.
• Develop and implement individualized care plans tailored to each member's clinical needs.
• Monitor patient progress through ongoing assessments and follow-up interactions.
• Collaborate within established protocols using advanced telecommunication systems.
