Job summary

Qualifications

• Must reside in Oklahoma.
• Active RN, LPC, or LCSW license.
• Minimum 1 year of clinical experience.
• 2+ years in care management.
• Experience coordinating services for Medicaid/Medicare enrollees.
• Knowledge of community health and social service agencies.
• Proficiency in Microsoft Office (Word/Excel/PowerPoint).

Responsibilities

• Employ strategies to manage enrollees' health needs.
• Monitor assessments to ensure progress toward outcomes.
• Identify and resolve barriers hindering care and progress toward outcomes.
• Create enrollee care plans aligned with objectives.
• Perform telephonic and face-to-face assessments of needs.
• Guide members to appropriate resources for wellbeing.
• Collaborate with providers and community services.
• Follow established guidelines/procedures.
• Possible travel to Humana Oklahoma office for meetings and training.
