Job summary

Molina Healthcare is hiring a remote RN Care Manager for our Washington State Health Plan. This role is remote with a WA preference and Pacific Standard Time hours.

Telephonic case management and transition-of-care support are provided, with travel, weekends, night shifts, or direct patient care not required. Provides care coordination across the care continuum in collaboration with a multidisciplinary team to improve outcomes and manage costs.

Qualifications

• RN with active license in state of practice
• 2+ years healthcare or care management experience
• Valid driver’s license and transportation for travel
• Knowledge of EMR and HIPAA
• Ability to work remotely and PST hours

Responsibilities

• Complete comprehensive assessments per timelines and identify candidates for care management
• Develop and implement care coordination plan with member and care team
• Conduct telephonic, in-person, or home visits as needed
• Monitor care plan effectiveness and document interventions
• Maintain ongoing member caseload and outreach
• Promote integration of behavioral health, LTSS, and community resources
• Facilitate ICT meetings and collaboration
• Educate and motivate members using MI and Molina guides
• Address barriers and provide care coordination to members
• Support peers with consultation when needed
• Handle complex cases and medication reconciliation when assigned
• Travel up to 25–40% as required by state/contractual needs
