Job summary

Molina Healthcare is hiring a remote RN Care Manager for our Washington State Health Plan. This role is remote with a preference for candidates in Washington state and requires working Pacific Standard Time hours.

The RN will support Medicare members through telephonic case management, coordination of care, and collaboration with a multidisciplinary team to achieve quality, cost-effective outcomes. Some travel may be required for member visits and program initiatives.

Qualifications

• RN license active and unrestricted in state of practice.
• Minimum 2 years healthcare experience, preferably in care management.
• Experience with EMR/HIPAA compliance and patient data handling.
• Ability to work independently and manage multiple priorities.
• Strong communication and problem-solving abilities.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with the member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor the care plan, document interventions, and adjust as needed.
• Maintain a regular member caseload and outreach.
• Facilitate integration of behavioral health, LTSS, and community resources.
