Molina Healthcare is seeking a remote RN Care Manager for our Washington State Health Plan. This role is remote with a preference for WA candidates and Pacific Standard Time hours.

It supports Medicare members via telephonic case management and transition of care, with no travel for nights, holidays, or weekends. The incumbent will coordinate across a multidisciplinary team, monitor progress toward outcomes, and contribute to cost-effective member care.

Qualifications

• At least 2 years healthcare experience, care management preferred.
• RN license must be active and unrestricted in state of practice.
• Valid driver's license and reliable transport for state requirements.
• Knowledge of EMR and HIPAA compliance.
• CCM certification is a preferred qualification.

Responsabilités

• Conduct telephonic, face-to-face or home visits as required.
• Develop and implement care coordination plans with the care team.
• Monitor care plans, document interventions and outcomes.
• Maintain ongoing member caseload and outreach.
• Promote integration of behavioral health and LTSS resources.
• Facilitate ICT meetings and collaborations.
• Educate and motivate change using motivational interviewing.
• Identify and address barriers to care and provide resources.
