Molina Healthcare is seeking a remote RN Care Manager to support our Washington State Health Plan. The role is remote with a preference for WA state residents and operates on Pacific Standard Time hours.

The RN will conduct telephonic case management and transition-of-care activities for Medicare members, collaborate with a multidisciplinary team, and maintain a caseload while traveling up to 25-40% as needed by state requirements.

Qualifications

• 2+ years experience in health care, preferably in care management.
• RN license active and unrestricted.
• Valid driver's license and reliable transportation for travel if required.
• Understanding of EMR and HIPAA.
• Knowledge of community resources.
• Ability to work independently and manage time.

Responsibilities

• Complete comprehensive member assessments per timelines.
• Develop and implement care coordination plans.
• Conduct telephonic, face-to-face, or home visits as needed.
• Monitor and document progress toward goals.
• Maintain ongoing member caseload and outreach.
• Coordinate services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and provide solutions.
• Possibly provide consultation to peers and manage complex cases.
• May conduct medication reconciliation as needed.
