Qualifications

Responsibilities

• Perform comprehensive member assessments within regulatory timelines and identify care management eligibility.
• Develop and execute coordinated care plans with members, caregivers, and providers.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor and document care plan progress, adapt as necessary to achieve goals.
• Manage an ongoing member caseload with proactive outreach.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
