Molina Healthcare is seeking a remote RN Care Manager to support Washington State Health Plan. The role is remote with preference for WA candidates and Pacific Time hours.

This position focuses on telephonic case management and transition-of-care for Medicare members, with no travel, weekends, holidays, or night shifts. You will collaborate with multidisciplinary teams to coordinate member care, perform assessments, and develop care plans, ensuring progress toward outcomes.

Qualifications

• Active RN license in state of practice.
• At least 2 years in healthcare or care management.
• Experience with telephonic case management preferred.
• Knowledge of EMR and HIPAA required.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines.
• Develop and implement care coordination plans with member, caregiver, physician and team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and cross-team collaboration.
• Use motivational interviewing to educate and motivate change.
• Identify barriers to care and provide targeted assistance.
• May provide consultation to peers as needed.
• Handle complex cases and medication reconciliation as needed.
• Travel up to 25–40% depending on state/contract.
