Job summary

Molina Healthcare is seeking a remote RN Care Manager to support our Washington State Health Plan through telephonic case management and care coordination. The role is remote with a preference for WA-based candidates and operates on Pacific Standard Time hours.

The position focuses on Medicare members, coordinating care across providers, assessing needs, and guiding members toward desired outcomes via evidence-based practices and collaboration with the multidisciplinary team.

Qualifications

• RN license active and unrestricted in state of practice.
• Minimum 2 years in health care, preferably care management or related setting.
• Experience with EMR systems and HIPAA compliance.
• Strong communication and time-management abilities.
• Ability to work independently in a remote, PST-hour environment.

Responsibilities

• Conduct comprehensive member assessments per timelines and identify who qualifies for care management.
• Develop and implement care coordination plans with the member and care network.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document interventions and outcomes.
• Maintain ongoing member caseload and regular outreach.
• Facilitate interdisciplinary care team meetings and collaboration.
• Provide education and motivational interviewing to support behavior change.
• Address barriers to care and coordinate resources.
