Job summary

Molina Healthcare is hiring a remote RN Care Manager for our Washington State Health Plan. The role is remote with a preference for candidates in Washington, working Pacific Standard Time hours.

This position supports Medicare members through telephonic case management and transition-of-care activities, with no current travel or night work. Required qualifications include an RN license, at least 2 years in health care or care management, and EMR/HIPAA proficiency.

Qualifications

• At least 2 years experience in health care, preferably in care management.
• Registered Nurse (RN) with active, unrestricted license.
• Valid driver’s license and reliable transportation for travel requirements.
• Understanding of EMR and HIPAA.
• Ability to work independently with minimal supervision.
• Strong time-management and communication skills.

Responsibilities

• Complete comprehensive assessments of members per timelines to identify care management eligibility.
• Develop and implement care coordination plans with member, family, and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and document progress; adjust interventions as needed.
• Maintain ongoing member caseload for outreach and management.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration with the care team.
• Address barriers to care and provide resources to members.
• May provide guidance to peers and support for complex cases.
• May conduct medication reconciliation as needed.
• Travel 25–40% based on state or contract requirements.
