Molina Healthcare is hiring a remote RN Care Manager for our Washington State Health Plan. This role is remote, with a preference for candidates in Washington state. It will operate on Pacific Standard Time hours to align with the plan's telephonic operations.

This role supports Medicare members through telephonic case management and transition-of-care functions, with no current travel or night work. Collaboration with multidisciplinary teams is essential to ensure quality, cost-effective member

Qualifikationen

• 2+ years experience in health care, preferably in care management.
• RN license must be active and unrestricted.
• Knowledge of EMR systems and HIPAA.
• Strong verbal and written communication skills.
• Excellent time-management and prioritization.

Aufgaben

• Complete comprehensive member assessments within required timelines.
• Develop and implement care coordination plans with the member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and update care plans; document interventions and outcomes.
• Maintain ongoing member caseload and outreach.
• Coordinate services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and interdisciplinary collaboration.
• Address barriers to care and provide necessary resources.
