Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) to work from home, serving Medicare/Medicaid and group health plan members. The role emphasizes telephonic health assessments, care planning, and chronic condition management.

Requires active RN license in state of residence and Compact RN eligibility, with 2+ years nursing experience. Seasonal work-from-home opportunity with starting pay of $37.00 per hour.

Qualifications

• Active, unrestricted RN licensed in state of residence.
• Compact RN license permitting practice in applicable compact states.
• Minimum 2 years clinical nursing experience (3+ preferred).
• Strong clinical judgment, critical-thinking and problem-solving.
• Ability to work independently in a remote environment.
• Proficiency in Microsoft Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Navigate healthcare benefits and care options with members and caregivers.
• Assess factors affecting health and well-being; collaborate with multidisciplinary teams.
• Promote self-management through education and resources.
• Coordinate care activities among members, caregivers, and providers.
• Document interactions, assessments, and outcomes accurately in systems.
