Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) for a seasonal, work-from-home opportunity. This role focuses on telephonic health assessments, care planning, and coaching for Medicare, Medicaid, and group health plan members.

The RN must be licensed in respective states, hold a Compact RN license, and bring 2+ years of clinical nursing experience. Remote work requires strong judgment and EMR proficiency.

Qualifications

• Active, unrestricted Registered Nurse license in the state of residence.
• Active Compact RN license allowing practice in applicable compact states.
• Minimum 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking and problem-solving skills.
• Ability to work independently in a remote environment with nursing judgment.
• Proficiency with Word, Excel and Outlook; navigate multiple EMR systems.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic conditions.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning with DME, home health, and appointments.
• Help members navigate benefits, services and care options.
• Assess factors affecting health and well-being; collaborate with multidisciplinary teams.
• Promote self-management with education and resources tailored to goals.
• Coordinate care activities among members, caregivers, providers, and care team.
• Document interactions, assessments and outcomes accurately in systems.
