Job summary

Sagility is seeking an experienced Senior Care Specialist (Registered Nurse) to work from home in the United States. This seasonal role involves telephonic health assessments, care coordination, and patient education, with a tentative end date in June 2027.

Responsibilities include developing individualized care plans, supporting transitions of care, and collaborating with multidisciplinary teams to optimize member outcomes. Prior nursing judgment and remote-work capability are essential.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and in a remote environment.
• Intermediate to advanced computer proficiency including Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education, encouragement, and support.
• Use clinical nursing judgment to assess member needs and coordinate interventions.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Coordinate care and condition-management activities across care teams.
