Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) for a work-from-home seasonal opportunity. You will conduct telephonic health assessments, develop care plans, and coordinate care with a multidisciplinary team to support members with chronic conditions.

The role requires an active RN license in the state, a Compact license, and 2+ years of clinical nursing experience. Excellent communication and computer skills are essential.

Qualifications

• Active, unrestricted RN license in the resident state.
• Compact RN license permitting practice in applicable states.
• Minimum 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Proficient with Microsoft Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for members.
• Develop actionable, individualized care plans based on risks.
• Establish health coaching relationships and educate on chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions.
• Coordinate transitions of care and arrange DME/home health services.
• Help members navigate benefits and care options.
• Assess environmental and psychosocial factors impacting health.
• Collaborate with multidisciplinary teams to support care.
• Promote self-management with education and resources.
• Document interactions accurately in systems.
