Job summary

Sagility is seeking a Senior Care Specialist (RN) to perform telephonic health assessments for Medicare/Medicaid and group plans from a work-from-home setting. This seasonal assignment ends June 2027 and offers starting pay of $37.00 per hour, with daily pay options.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Strong clinical judgment and problem-solving abilities.
• Ability to work independently in a remote environment.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Coordinate interventions and transitions of care, including DME and home health services.
• Navigate members and caregivers through benefits, services, and care options.
