Sagility is hiring a Senior Care Specialist (Registered Nurse) to deliver telephonic health assessments and care coordination from home. This seasonal opportunity supports Medicare/Medicaid populations and requires an active RN license and compact license.

Qualifications

• Active, unrestricted RN license.
• Compact RN license required.
• Strong clinical judgement and remote work ability.
• Proficiency with Word, Excel, Outlook.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, and provider appointments.
• Help members navigate healthcare benefits, services, and available care options.
• Assess physical, environmental, and psychosocial factors that may affect health and well-being.
• Collaborate with multidisciplinary teams including social workers, dietitians, pharmacists, providers, and other healthcare professionals.
• Promote member self-management by providing education and resources.
• Coordinate care and condition-management activities among members, caregivers, providers, and care team.
• Document member interactions, assessments, care plans, interventions, and outcomes accurately in applicable systems.
