Sagility is seeking a Senior Care Specialist (Registered Nurse) for a seasonal, work-from-home role. You will conduct telephonic health assessments, develop personalized care plans, and coach members on chronic condition management from your remote setting in Ohio/USA.

The role requires an active RN license, compact-state license, and 2+ years of clinical nursing experience with strong communication skills. Starting pay is $37.00 per hour with daily pay options.

Formación

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.

Responsabilidades

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME coordination.
• Help members navigate healthcare benefits, services, and care options.
• Assess environmental factors affecting health and well-being.
• Collaborate with multidisciplinary teams including social workers and providers.
• Promote member self-management with education and resources.
• Coordinate care and condition-management activities among care team members.
