Sagility is seeking an experienced Senior Care Specialist (Registered Nurse) to perform telephonic health assessments and develop individualized care plans from a work-from-home setting in Ohio. This seasonal opportunity targets experienced RNs with a strong clinical background and remote-care capabilities.

You will coordinate transitions of care, provide education, and collaborate with a multidisciplinary team to support members’ health goals and optimize outcomes.

Qualifikationen

• Active, unrestricted RN license in applicable state of residence.
• Active Compact RN license permitting practice in compact states.
• Minimum 2 years clinical nursing experience; 3+ preferred.
• Strong clinical judgment, critical-thinking and problem-solving.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency with MS Word/Excel/Outlook.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on identified risks.
• Establish health coaching relationships and provide education and support.
• Assess needs with nursing judgment and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning with DME and home health services.
• Help members navigate benefits and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams including social workers and pharmacists.
• Promote self-management with education and resources.
• Coordinate care among members, caregivers, and providers.
• Document interactions, assessments, and outcomes accurately in systems.
