Responsibilities

• Conduct telephonic health assessments for Medicare/Medicaid/group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships with members and provide education and support.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Coordinate transitions of care and arrange DME, home health, and provider appointments.
• Navigate healthcare benefits and options with members and caregivers.
• Document interactions, assessments, and outcomes accurately in systems.
