Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) for a work-from-home seasonal opportunity. The role focuses on telephonic health assessments, care plan development, and health coaching for Medicare/Medicaid members. Remote work is supported with a flexible, patient-centered approach.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for members across Medicare, Medicaid, and group plans.
• Develop individualized, measurable care plans based on risks.
• Establish health coaching relationships and provide education and support for chronic conditions.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning with DME and home health services.
• Help members navigate benefits and available care options.
• Assess environmental and psychosocial factors affecting health.
• Collaborate with social workers, dietitians, pharmacists, and providers.
• Promote self-management with tailored education and resources.
• Coordinate care activities among members, caregivers, and the care team.
• Document interactions, assessments, and outcomes in systems.
