Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) to support members remotely. The role focuses on telephonic assessments, care planning, and education to manage chronic conditions. This seasonal, work-from-home opportunity emphasizes independent nursing judgment and seamless coordination with a multidisciplinary team.

Starting pay is $37.00 per hour, with daily wage access. Location is Work@Home Ohio, United States. Equal opportunity employer.

Qualifications

Active RN license in home state.
Compact RN license permitting practice in applicable states.
Minimum of 2 years clinical nursing experience; 3+ preferred.
Ability to work independently in a remote environment.
Proficient computer skills: Word, Excel, Outlook.

Responsibilities

Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
Develop actionable, individualized care plans based on member needs and risks.
Establish health coaching relationships with members and provide education and support.
Use nursing judgment to assess needs and coordinate interventions for acute situations.
Coordinate transitions of care and discharge planning, including DME and home health services.
Assist members in navigating benefits, services, and care options.
Assess factors that affect health and well-being; document interactions accurately.
