Qualifikationen

• Bachelor’s degree required in public health, health administration, public policy, business administration, or related field.
• 5 years of experience in grant management, contract oversight, program administration, or related work.
• 3 years of experience with healthcare organizations, rural communities, or public health systems.
• 2 years of experience with compliance, budgeting, and reporting requirements for state or federal grants.
• Ability to manage multiple projects with strong attention to detail.
• Excellent communication, facilitation, and relationship-building skills.
• Ability to work independently as a remote field-based contractor.
• Must reside within the assigned RHTP region and be willing to travel regularly within the region.
• Experience with CMS-funded programs or federal cooperative agreements.
• Familiarity with Indiana's rural health landscape and regional coalitions.
• Experience collaborating with statewide associations or multi-stakeholder coalitions.
• Knowledge of rural health delivery, care coordination, or health system transformation.

Aufgaben

• Serve as the primary regional contract manager for organizations receiving RHTP funding.
• Monitor compliance with state and federal requirements, including expenditures, budget adherence, and documentation standards.
• Support IDOH in ensuring regional coalitions meet milestones, deliverables, and timelines.
• Identify risks and elevate concerns as appropriate; maintain audit-ready documentation.
• Collaborate with statewide Technical Assistance Providers to ensure aligned support for regional coalitions.
• Coordinate regional data collection, performance tracking, and CMS reporting requirements.
• Serve as day-to-day liaison between IDOH and regional coalitions, including hospitals and clinics.
• Assist regional partners in interpreting state guidance, funding requirements, and program expectations.
• Track progress, identify barriers, and support problem-solving to maintain momentum.
