Job summary

CAI is seeking a Regional Grant Coordinator to support Indiana’s 5-year Rural Health Transformation Program (RHTP) and MRIHA Regional Grants. The role is full-time, long-term, and hybrid in Indianapolis, IN, with remote work flexibility and no sponsorship.

You’ll manage regional contracts, ensure CMS reporting compliance, and collaborate with state providers to advance rural health goals. The ideal candidate has a public health or related degree and strong experience in grant management,

Qualifications

• Bachelor’s degree required in public health, health administration, public policy, business administration, or related field.
• 5 years of grant management, contract oversight, program administration or related work.
• 3 years working with healthcare organizations, rural communities, or public health systems.
• 2 years of experience with compliance, budgeting, and reporting for state or federal grants.
• Ability to manage multiple projects with strong attention to detail.
• Excellent communication, facilitation, and relationship-building skills.
• Ability to work independently as a remote field-based contractor.
• Must reside within the assigned RHTP region and be willing to travel regularly within the region.
• Experience with CMS-funded programs or federal cooperative agreements.
• Familiarity with Indiana’s rural health landscape (hospitals, FQHCs, EMS, community orgs).
• Experience collaborating with statewide associations or multi-stakeholder coalitions.
• Knowledge of rural health delivery, care coordination, or health system transformation.

Responsibilities

• Regional Grant & Contract Management for RHTP funding.
• Monitor compliance with state/federal requirements and documentation standards.
• Support IDOH in milestones, deliverables, and timelines.
• Coordinate with Technical Assistance Providers and regional coalitions.
• Oversee CMS reporting requirements including data collection and metrics.
• Engage regional stakeholders and maintain strong IDOH communication.
• Assist partners in interpreting state guidance and funding requirements.
• Track progress, identify barriers, and support problem-solving.
• Contribute to statewide consistency using shared tools and templates.
• Participate in required trainings and statewide meetings.
