CAI is seeking a Regional Grant Coordinator to support Indiana’s Rural Health Transformation Program MRIHA regional grants. This full-time, long-term contract role is remote with hybrid options and requires managing regional contracts, compliance, and CMS reporting.

The successful candidate will collaborate with statewide partners, track milestones, and ensure audit-ready documentation while traveling within the assigned region as needed.

Qualifikationen

• Bachelor’s degree in public health, health administration, public policy, business administration, or related field.
• 5 years of experience in grant management, contract oversight, program administration, or related work
• 3 years of experience working with healthcare organizations, rural communities, or public health systems
• 2 years of experience with compliance, budgeting, and reporting requirements for state or federal grants
• Ability to manage multiple projects simultaneously with strong attention to detail
• Excellent communication, facilitation, and relationship-building skills
• Ability to work independently as a remote field-based contractor
• Must reside within the assigned RHTP region and be willing to travel regularly within the region
• Experience with CMS-funded programs or federal cooperative agreements
• Familiarity with Indiana's rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations
• Experience collaborating with statewide associations or multi-stakeholder coalitions
• Knowledge of rural health delivery, care coordination, or health system transformation

Aufgaben

• Serve as the primary regional contract manager for organizations receiving RHTP funding.
• Monitor compliance with state and federal requirements, including expenditures, budget adherence, and documentation standards.
• Support IDOH in ensuring regional coalitions meet milestones, deliverables, and timelines in the workplan.
• Identify risks and compliance concerns and elevate to IDOH as appropriate.
• Maintain audit-ready documentation and ensure use of state-approved templates and reporting systems.
• Collaborate with Technical Assistance Providers to ensure aligned regional support.
• Participate in regional meetings, consortium meetings, workgroups, and development sessions.
• Coordinate regional data collection, performance tracking, and CMS reporting requirements.
• Ensure timely submission of regional reports to IDOH for state/federal cycles.
• Serve as liaison between IDOH and regional coalitions, hospitals, clinics, EMS, and community partners.
• Promote broad representation of rural populations and local needs.
• Maintain communication with IDOH’s Regional Grant Initiative Team and participate in statewide meetings.
