Job summary

CAI is hiring a Regional Grant Coordinator to support Indiana’s Rural Health Transformation Program (RHTP) and the MRIHA Regional Grants. The role is full-time, long-term, and hybrid in Indianapolis, IN, with remote work options in a contract setup.

The candidate will manage regional contracts, ensure compliance with state and CMS reporting requirements, and engage with hospitals, EMS, and community partners to advance rural health objectives. Must reside in the RHTP region and travel as needed.

Qualifications

• Bachelor’s degree in public health, health administration, public policy, business administration, or related field.
• 5 years of experience in grant management, contract oversight, program administration, or related work.
• 3 years of experience working with healthcare organizations, rural communities, or public health systems.
• 2 years of experience with compliance, budgeting, and reporting requirements for state or federal grants.
• Ability to manage multiple projects simultaneously with strong attention to detail.
• Excellent communication, facilitation, and relationship-building skills.
• Ability to work independently as a remote field-based contractor.
• Must reside within the assigned RHTP region and be willing to travel regularly within the region.
• Experience with CMS-funded programs or federal cooperative agreements.
• Familiarity with Indiana's rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations.
• Experience collaborating with statewide associations or multi-stakeholder coalitions.
• Knowledge of rural health delivery, care coordination, or health system transformation.

Responsibilities

• Regional Grant & Contract Management: Serve as the primary regional contract manager for organizations receiving RHTP funding, monitor compliance with state and federal requirements, and maintain audit-ready documentation.
• Coordination with Technical Assistance Providers: Collaborate with statewide Technical Assistance Providers and participate in regional meetings and workgroups.
• Oversight of CMS Reporting Requirements: Coordinate regional data collection, performance tracking, and timely submission of regional reports to IDOH.
• Regional Stakeholder Engagement & Communication: Act as the day-to-day liaison between IDOH and regional coalitions, promote broad representation, and maintain strong communication channels.
• Program Implementation Support: Assist regional partners in interpreting state guidance and funding requirements, track progress, and support problem-solving.
