Job summary

Mira Mace is seeking a remote Patient Navigator on a 1099 independent contractor basis to support the Rapid Response Team. The role focuses on non-clinical navigation, removing barriers to care, and documenting progress for each case.

Applicants should have an active LPN/LVN license or CHW certification, 2+ years in patient-facing support, and a strong ability to communicate clearly while maintaining HIPAA-compliant confidentiality.

Qualifications

• An active LPN/LVN license; or CHW certification; or equivalent community health experience.
• 2+ years of patient-facing support experience (care coordination, case management, member advocacy, health coaching, benefits navigation, or similar).
• Working knowledge of the U.S. healthcare system and insurance.
• Comfort working independently, managing multiple urgent cases at once, and maintaining strong documentation.
• Reliable computer, internet, and a quiet space for patient calls.

Responsibilities

• Take cases that reach you already triaged: confirm essentials and assign the case to you.
• Read the case and build an execution plan: what must happen, in what order, how fast.
• Work the plan using provided resource directories; find your own when needed.
• Keep ops updated: progress, blockers, and what you need to move faster.
• Call the patient to share progress and confirm details.
• Document every interaction in our platform for seamless handoffs across a 24/7 team.
