Job summary

Vivo Care is seeking a Care Navigator to build trust-based relationships with patients, triage vital signs via a Remote Care Platform, and coach patients toward healthier behaviors. The role focuses on RPM data, wellness calls, and collaboration with the care team to set patient-centered goals.

The ideal candidate holds an unrestricted RN/LPN/LVN license, prefers experience in remote triage and Medicare patients, and can work MST hours.

Qualifications

• Unrestricted RN/LPN/LVN license in a compact state.
• 1-3 years of clinical experience, patient management, or disease management.
• Experience with Medicare patients.
• Experience performing virtual visits with patients and telephonic care management.
• Motivational Interviewing/Health Behavior Change experience is a plus.
• Spanish speaking a plus.

Responsibilities

• Manage physiological markers with clinical appropriateness (blood pressure, weight, glucose).
• Meet team goals and standards outlined metrics.
• Significantly impact longitudinal patient engagement in RPM programs.
• Provide preventive health and disease management education and coaching.
• Perform monthly wellness calls with assigned patients.
• Lead collaborative wellness calls to define health goals with the care team.
• Manage patient messaging and alerts.
• Direct patients to treating physician for routine questions.
• Meet patient engagement program goals.
• Follow escalation pathways for urgent care needs.
