This role requires CMA or RMA certification and BLS, with HIPAA-compliant remote work setup. Eligible for performance bonuses and paid training. This position offers a fully remote schedule with no weekends.

Qualifications

• Requires CMA or RMA certification and BLS
• Experience in patient care coordination or care management
• Ability to document accurately in EHR and navigate multiple systems
• Excellent communication and time-management skills

Responsibilities

• Make proactive outbound patient calls to discuss medications, symptoms, and care needs
• Coordinate with providers, pharmacies, and other healthcare resources
• Identify concerns and communicate them to the clinical team
• Document patient interactions accurately in the EHR
• Maintain high-quality and productive patient encounters, targeting at least 2 billable visits per hour
• Work independently in a home-based setting with HIPAA-compliant workspace
