Job summary

MiraMace is a remote, U.S.-based provider delivering Medicare navigation services to patients. As a Care Advocate, you’ll guide patients through care plans, help schedule appointments, and connect them with community resources.

You’ll support multiple patients, ensure smooth communications with providers, and maintain non-clinical documentation under HIPAA guidelines. This is a 1099 contractor role with 20–40 hours weekly.

Qualifications

• CHW certification or equivalent community health/patient-navigation experience.
• 2+ years of patient- or community-facing support experience.
• Working knowledge of the U.S. healthcare system and Medicare.
• Comfort working independently, managing a caseload, and maintaining strong documentation.
• Reliable computer, internet, and a quiet space for patient calls.

Responsibilities

• Serve as the primary point of contact for patients after intake.
• Help patients understand and act on their care plan, schedule appointments, and find in-network providers.
• Resolve navigation friction: portals, referrals, appointment logistics, forms, and provider communications.
• Identify practical and social-needs support (food, housing, transportation) and connect to resources.
• Coordinate with clinical team while providing non-clinical support only.
• Document every patient interaction and support activity in the platform on time.
• Maintain confidentiality and HIPAA compliance.
