Devoted Health is seeking a Social Work Case Manager to support our Primary Care program. You will deliver telehealth visits, assess psychosocial needs, and collaborate with clinicians to coordinate care for high-needs patients in a home-based setting.

Qualifications

• MSW and active social work license.
• Minimum of 2 years expertise in the medical field.
• Comfortable with technology and a fast-paced environment.
• Self-starter able to succeed in a work-from-home setting.
• Strong interpersonal and communication skills.

Responsibilities

• Telephonic and video visits with patients and caregivers.
• Establish trust and build strong relationships with patients.
• Collaborate with clinicians, nurses, and care coordinators to ensure patient-centered care.
• Conduct Social Work Case Management interventions to maximize program effectiveness.
• Perform psychosocial screenings with personalized care plans.
