Devoted Health is seeking a Palliative Care Social Work Case Manager to support patients and families remotely, guiding goals of care and coordinating with clinicians, nurses, and care coordinators. You will conduct psychosocial assessments, develop individualized care plans, and provide counseling to reduce barriers to care.

The role emphasizes compassionate, patient-centered care for individuals in the final year of life, with proficiency in video/phone communication and strong collaboration

Qualifications

• Master’s degree in social work and active license.
• 2+ years in palliative or hospice, leading goals of care conversations.
• Credential in hospice/palliative social work or willing to obtain within 1 year.
• Comfortable with technology in a fast-paced environment.
• Work-from-home capability and strong communication skills.

Responsibilities

• Telephonic and video visits with patients and caregivers.
• Conduct psychosocial assessments and create personalized care plans.
• Coordinate interdisciplinary team for cohesive patient-centered care.
• Provide emotional support and caregiver assistance.
• Lead goals of care and advanced care planning discussions.
