Job summary

Devoted Health in the United States is seeking a Complex Care Social Work Case Manager to support a multidisciplinary team in coordinating services for high-need patients. Qualifications

• MSW degree and valid state social work license.
• Minimum 3 years in medical social work, case management, or care coordination.
• Bilingual Spanish preferred.

Responsibilities

• Establish trust with patients telephonically and virtually.
• Coordinate care with interdisciplinary team to ensure cohesive, patient-centered care.
• Identify social barriers and align resources for effective care planning.
• Facilitate conversations about end-of-life planning when appropriate.
• Support caregivers and monitor outcomes with the Behavioral Health Team.
