Job summary

Devoted Health is seeking a Palliative Care Social Work Case Manager to provide telephonic and video-based support to patients and families, focusing on goals of care and advanced care planning in a remote US setting.

You will conduct psychosocial assessments, develop individualized care plans, coordinate with the care team, and assist with grief and bereavement support while navigating healthcare systems with empathy and professionalism.

Qualifications

• Master’s degree in social work and an active license.
• Minimum 2 years in palliative or hospice care with goals of care experience.
• Comfortable with technology and fast-paced, work-from-home environment.
• Strong communication and interpersonal skills; bilingual abilities are a plus.

Responsibilities

• Provide telephonic and video visits to patients and caregivers.
• Lead goals of care conversations and advanced care planning.
• Coordinate interdisciplinary team care and document in EHR.
• Offer emotional support and navigate family dynamics and resources.
