Advocate Health is seeking a Care Coordination professional for Value Enablement Services - Value Based: Patient Outreach. This full-time role offers a remote schedule with occasional onsite meetings in NC or GA, Monday–Friday 8:00 AM-4:30 PM, and no relocation.

Responsibilities include coordinating case intake, maintaining guidelines, onboarding employer groups, conducting care gap outreach, training staff, producing reports, and managing materials for Population Health initiatives.

Qualifications

• High School Diploma required.
• Typically requires 5 years of experience in population health management or related field.
• Proficiency with ICD-10, CPT and HCPCS coding.

Responsibilities

• Coordinate intake of cases to expedite Care Coordination and triage to resources.
• Research, organize, and maintain the team's resource library and policies.
• Facilitate onboarding of new employer groups with onsite Nurse and sales.
• Coordinate care gap outreach and verify lists and interventions.
• Assist in training staff on systems, health plans, and procedures.
• Develop and generate reports; summarize data for staff and partners.
• Coordinate materials for Population Health activities.
• Maintain department files and records; improve documentation.
• Perform routine administrative duties and office support tasks.
