Job summary

Southeast Medical Group is seeking a Population Health Care Coordinator to support population health initiatives, identify and close care gaps, coordinate preventive services, and facilitate patient outreach in a remote or hybrid work setting.

The role partners with primary care teams, nurses, and quality leadership to improve patient engagement, preventive care compliance, and quality measures while leveraging EHRs and dashboards to monitor progress and ensure accurate documentation.

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's degree in healthcare-related field preferred.
• CMA/RMA or similar certification preferred but not required.
• 1+ years of experience in a medical office or care coordination preferred.
• Experience with EHR and population health/quality reporting preferred.

Responsibilities

• Identify care gaps using registries and dashboards.
• Proactive patient outreach for appointments and screenings.
• Coordinate with practice teams to complete identified care needs.
• Manage AWV scheduling and documentation.
• Support transitions of care and post-discharge follow-up.
• Prepare population health reports and dashboards.
