Qualifications

• Must reside in North Carolina.
• Completion of an accredited nursing program.
• Registered Nurse license in the state of practice.
• BLS for Healthcare Providers certification by the American Heart Association.
• Prior Ambulatory, Family Medicine and NC Medicaid experience preferred.
• Care Management certification preferred.

Responsibilities

• M-F, 8a-5p; remote.
• Coordinate and manage ambulatory care for patients.
• Collaborate with physicians and care teams to ensure continuum of care.
• Support NC Medicaid initiatives and patient education.
