Job summary

Community Care of North Carolina Inc. is seeking a Member Care Coordinator to support the care management team in improving patient outcomes and care coordination across NC.

This non-clinician role will help with referrals, scheduling, health system navigation, and data documentation while collaborating with providers and QI teams. You will work with regional teams, may be remote within regions, and on-site at CCPN practices as needed.

Qualifications

• High school diploma or GED required.
• ~2-4 years minimum experience in a health care setting preferred.
• Excellent communication and organizational skills.
• Knowledge of medical terminology and health care systems beneficial.

Responsibilities

• Receive and document referrals into the care management platform.
• Outreach, engagement, and scheduling of members for Care Managers.
• Assist with mailing educational materials, consent forms or other documents to members.
• Assist with referrals and coordination of services as directed by Care Management/QA teams.
• Support care coordination and navigation across health care systems.
• Ensure documentation and reporting align with HIPAA and privacy policies.
• Travel may be required within region and/or the State.
