Job summary

Advocate Health is seeking a detail-oriented professional to support Care Coordination operations in a structured office setting. The role involves facilitating medical reviews, verifying eligibility, data collection, and triaging to appropriate resources.

You will expand knowledge through literature reviews, support onboarding for employer groups, and help implement care gap outreach while ensuring accurate reporting and data integrity.

Qualifications

• High School Diploma or equivalent.
• Typically requires 5 years of experience in population health or related fields.
• Proficiency in ICD-10, CPT and HCPCS coding preferred.

Responsibilities

• Coordinate intake of cases to expedite Care Coordination and triage to appropriate resources.
• Research and maintain the team's library of guidelines and policies; support literature reviews.
• Assist with onboarding materials and reporting tools for new employer groups.
• Coordinate care gap outreach and document interventions and outreach lists.
• Train staff in referral/authorization processes and department procedures.
• Develop and summarize data for reporting and analysis for staff and partners.
• Order and stock materials to support Population Health activities.
• Maintain department files and records; perform routine admin duties.
• Screen calls, route mail, type documents, and manage office equipment.
