Southeast Medical Group seeks a Population Health Care Coordinator to support population health initiatives, close care gaps, and coordinate preventive services. You will work with primary care teams, nurses, and quality leadership to improve patient engagement and outcomes.

The role involves AWV coordination, medical records management, and transitions of care, with emphasis on HIPAA compliance and accurate documentation.

Qualifications

• High school diploma or equivalent required.
• Associate's or bachelor's degree in healthcare administration, public health, health sciences, or related field preferred.
• Certified Medical Assistant (CMA/RMA) or similar healthcare certification preferred, but not required.
• One or more years of experience in a medical office, healthcare administration, patient scheduling, care coordination, or related healthcare setting preferred.
• Experience using Electronic Health Records (EHR) required or demonstrated ability to become proficient quickly.

Responsabilités

• Care Gap Identification & Closure: review registries and dashboards to identify gaps and coordinate services.
• AWV Coordination: identify beneficiaries due for AWVs and schedule with primary care practices.
• Medical Records Management & Quality Documentation: obtain and organize external records in the EHR.
• Transitions of Care Support: monitor admissions/discharges and coordinate follow-up.
• Population Health Operations & Reporting: assist with population health reports and dashboards.
