Southeast Primary Care Partners is seeking a Population Health Care Coordinator (Quality & Care Gap Specialist) to support assigned practice sites with remote or hybrid work options. The role focuses on identifying and closing care gaps, coordinating Annual Wellness Visits, managing medical records, and supporting transitions of care.

The position requires EHR proficiency, strong organizational and communication skills, and familiarity with population health, CMS/ACOs, and HIPAA compliance.

Qualifications

• 1+ years in medical office or healthcare administration preferred.
• Experience with EHR systems required or demonstrated ability to learn quickly.
• Familiarity with population health, quality reporting, and value-based care programs preferred.

Responsabilités

• Care Gap Identification & Closure across preventive, chronic, and quality-related care gaps.
• Review patient registries, EHR reports, and population health dashboards to identify gaps.
• Proactive patient outreach to facilitate appointments, screenings, and preventive services.
• Coordinate with practice teams to ensure timely completion of identified care needs.
• Track outreach attempts, patient responses, and completed care gap activities.
• Assist with reconciling documented services and improving clinical documentation for quality measures.
• AWV Coordination: identify beneficiaries due for AWVs and schedule with primary care practices.
• Maintain scheduling records and follow-up on canceled or missed AWVs.
• Request and obtain external medical records; review for completeness and relevant documentation.
• Upload, index, and organize records within the EHR per procedures; ensure HIPAA compliance.
• Support transitions of care with post-discharge follow-ups and coordination.
• Assist with population health reporting and dashboard validation.
