Qualifications

• Must reside in North Carolina.
• Completion of an accredited or approved nursing program.
• Registered Nurse license issued by the state of practice.
• Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Ambulatory/family medicine and NC Medicaid experience preferred.
• Care Management certification preferred.
• BSN preferred.

Responsibilities

• Coordinate ambulatory care management and care coordination for Advocate Ambulatory Care Coordination and Management - Charlotte.
• Maintain M-F, 8am–5pm schedule, with remote work option as stated.
