Job summary

Sonexus Health is hiring a Care Coordinator for a 100% remote role in the United States. You will handle patient referrals, financial assistance applications, and follow-up communications while using our centralized platform.

Responsibilities include outbound calls, data entry, and coordinating with healthcare providers to move applications toward final decisions. High school diploma and Medicare/Medicaid or HUB experience are required; onboarding lasts 4–6 weeks, with virtual training.

Qualifications

• High School Diploma or equivalent is required.
• Must have Medicare/Medicaid experience within the last 3 years.
• Experience with HUB re-enrollment programs, pharmacy operations, or within an insurance company environment.
• Strong verbal and written communication, with a patient-centric approach.

Responsibilities

• Create and complete accurate patient referrals and financial assistance applications into the centralized platform.
• Place outbound calls for patient follow-ups, confirmations, and general updates.
• Resolve patient questions and concerns regarding their overall patient journey status.
• Enter highly detailed, accurate information into company proprietary software while conversing by telephone.
• Interact professionally with patient referral sources, including healthcare providers, to process new applicants.
• Maintain quality while providing empathetic, supportive patient experience and control the conversation.
• Steward patient accounts from initial contact through final approval or denial.
• Apply skills, training, and policies to complete routine tasks requiring basic problem resolution.
