Job summary

Cosán Group is hiring a Care Coordinator for a 100% remote US-based role. You will manage a patient panel, perform monthly outreach, and provide health coaching to improve outcomes while collaborating with clinical teams.

The ideal candidate has an MA-C or pharmacy certification, at least 1 year of clinical experience, and strong EMR navigation skills. A HIPAA-compliant home office and US work authorization are required, with a Monday-Friday schedule.

Qualifications

• Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C).
• OR Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills.
• Proficiency in multiple technology platforms and ability to navigate complex systems.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of patients and conduct required monthly outreach.
• Conduct 50-60 daily outbound calls to patients and provide health coaching.
• Maintain call quality standards and professional communication with patients.
• Collaborate with clinical teams to address medical and social needs.
• Close gaps in patient care through Chronic Care Management and related services.
• Document all patient interactions in real time while maintaining HIPAA compliance.
