Job summary

Lumicera is seeking a part-time Pharmacist, Patient Consultation Services to provide telephonic clinical consultations for specialty pharmacy patients. You will perform patient assessments, medication reconciliation, and patient education on medication use, adherence, and safety.

Two primary coverage models exist: 2x10-hour shifts weekly (Mon and another weekday, 8:30 AM–7:00 PM CT) and 5x4-hour shifts Mon–Fri (11:00 AM–3:00 PM CT). Additional scheduling may occur to meet high-volume needs.

Qualifications

• ACPE-accredited school graduate with PharmD or B.Pharm.
• Current pharmacist license in state of residence or eligible for reciprocity within 4 months.
• Experience counseling patients and documenting assessments per standards.
• Experience evaluating regimens for drug interactions and adherence barriers.
• Ability to participate in compliance programs and maintain patient privacy.

Responsibilities

• Conduct telephonic clinical consultations to support safe use of specialty medications.
• Perform patient assessments and medication reconciliation.
• Provide education on disease state management, administration, adherence, and side effects.
• Answer clinical questions from patients and caregivers.
• Escalate findings to promote continuity of care and patient safety.
• Document consultations and recommendations in pharmacy systems per policy.
• Maintain productivity, quality, and service level standards.
• Support quality improvement by reporting medication errors and safety concerns.
• Stay current on specialty medications and guidelines relevant to patients.
• Adhere to laws, regulations, and accreditation standards.
• Other duties as assigned.
