Job summary

Lumicera is seeking a Pharmacist, Patient Consultation Services for part-time telephonic clinical support in the United States. Coverage includes two shift models: 2x10-hour weekly and 5x4-hour weekly, with typical hours Monday–Friday.

The role conducts patient assessments, medication reconciliation, and education, interacting with patients, caregivers, pharmacists, technicians, and other healthcare professionals.

Qualifications

• Pharmacy degree from an ACPE-accredited school.
• Current, unrestricted Pharmacist license in the primary state of residence.
• Experience counseling patients and caregivers on medication therapy and disease state management.
• Experience documenting patient assessments, interventions, and medication-related recommendations.

Responsibilities

• Conducts telephonic clinical consultations to support safe and effective use of specialty medications.
• Performs patient assessments and medication reconciliation to identify adherence barriers and drug interactions.
• Provides education on disease state management, administration, storage, expectations, and side effects.
• Answers clinical questions from patients and caregivers to support informed treatment decisions.
• Documents consultations, assessments, interventions, and recommendations in pharmacy systems per policies and regulations.
• Maintains productivity, quality, and service level standards; supports quality improvement and patient safety initiatives.
