Job summary

Lumicera is seeking a Pharmacist, Patient Consultation Services on a part-time basis to support two coverage models: 2 x 10‑hour shifts (Mondays and another weekday, typically 8:30 AM–7:00 PM CT) and 5 x 4‑hour shifts (Mon–Fri, typically 11:00 AM–3:00 PM CT).

The Pharmacist conducts telephonic clinical consultations for specialty medications, performs patient assessments and medication reconciliation, provides education on disease state management and administration, and documents findings in

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a bachelor’s degree in Pharmacy or PharmD required.
• Current, unrestricted licensure as a Registered Pharmacist in the state of residence, or ability to gain reciprocity within 4 months.
• Experience counseling patients and caregivers regarding medication therapy and disease state management.
• Experience documenting patient assessments and interventions in pharmacy systems.

Responsibilities

• Conducts telephonic clinical consultations for specialty medications.
• Performs patient assessments and medication reconciliation to identify adherence barriers and drug interactions.
• Provides education on disease state management, medication administration, storage, treatment expectations, and side effects.
• Answers clinical questions from patients and caregivers to support informed treatment decisions.
• Identifies patient-specific clinical concerns and escalates findings to promote continuity of care and safety.
• Documents patient consultations, assessments, and recommendations in pharmacy systems per policies and regulations.
• Maintains productivity, quality, and service level standards through timely completion of consultations.
• Supports quality improvement by identifying and reporting medication errors and safety concerns.
