Job summary

Lumicera seeks a part-time Pharmacist, Patient Consultation Services, to provide telephonic clinical consultations for specialty medications and support patient care teams.

The role supports two primary schedules: 2 x 10-hour shifts per week (Mondays + another weekday) or 5 x 4-hour shifts (Mon-Fri, CT time). It involves patient assessments, education, and coordination across clinicians.

Qualifications

• ACPE-accredited pharmacy degree required.
• Current, unrestricted pharmacist license in state of residence or ability to reciprocity within 4 months.
• Experience counseling patients and caregivers on medication therapy.
• Experience documenting patient assessments and interventions in pharmacy systems.
• Experience evaluating medication regimens for interactions and adherence concerns.

Responsibilities

• Conduct telephonic clinical consultations to support safe use of specialty medications.
• Perform patient assessments and medication reconciliation to identify adherence barriers.
• Provide education on disease state management, administration, storage, and side effects.
• Answer clinical questions from patients and caregivers to support treatment decisions.
• Escalate findings through established processes to promote continuity of care and safety.
• Document consultations and interventions in pharmacy systems per policies.
• Maintain productivity and quality standards in patient consultations.
