Job summary

Lumicera is seeking a part-time Pharmacist, Patient Consultation Services to support telephonic clinical consultations for specialty medications. The role involves patient assessments, medication reconciliation, education on medication use, and collaboration with patients, caregivers, pharmacists, and healthcare professionals.

The position supports two shift models: 2 x 10-hour shifts per week (Mondays and another weekday) and 5 x 4-hour shifts per week (Mon-Fri), CT time zones.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a PharmD or B.Pharm.
• Current, unrestricted pharmacist license in the state of residence.
• Experience counseling patients and caregivers on medication therapy.
• Experience documenting patient assessments and interventions in pharmacy systems.
• Ability to adhere to regulatory standards and quality programs.

Responsibilities

• Conduct telephonic clinical consultations to support safe use of specialty meds.
• Perform patient assessments and medication reconciliation to identify adherence barriers.
• Provide education on disease state management, administration, storage, and side effects.
• Answer clinical questions from patients and caregivers to aid treatment decisions.
• Document consultations and clinical recommendations in pharmacy systems per policy.
