Job summary

Lumicera is seeking a Pharmacist, Patient Consultation Services to provide telephonic clinical consultations for specialty medications. The role performs patient assessments, medication reconciliation, and education for disease state management, adherence, and safety.

The position supports two primary shift models: 2 x 10-hour shifts weekly or 5 x 4-hour shifts weekly, with potential additional scheduling during high-volume periods. Remote work options apply with state licensure requirements.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a PharmD or bachelor’s degree in Pharmacy.
• Current, unrestricted pharmacist licensure in the state of residence, or ability to gain reciprocity within 4 months.
• Experience counseling patients and caregivers regarding medication therapy and disease state management.
• Experience documenting patient assessments and interventions in accordance with standards.
• Experience evaluating medication regimens for interactions and adherence barriers.

Responsibilities

• Conducts telephonic clinical consultations to support safe and effective use of specialty medications.
• Performs patient assessments and medication reconciliation to identify adherence barriers and potential concerns.
• Provides education on disease state management, medication administration, storage, and side effects.
• Answers clinical questions from patients and caregivers to aid treatment decisions.
• Identifies patient-specific concerns and escalates findings to promote continuity of care and safety.
• Documents consultations and interventions in pharmacy systems per policy and accreditation standards.
• Maintains productivity and service level standards through timely completion of consultations.
• Supports quality improvement by reporting medication errors and safety concerns.
• Maintains current knowledge of specialty meds, guidelines, and emerging therapies.
• Adheres to federal & state laws, regulations, and organizational policies governing specialty pharmacy practice.
• Other duties as assigned.
